When do muscles actually tear?

Most muscle injuries happen while the muscle is lengthening under load.

The simple answer

A pulled muscle is exactly what it sounds like: fibres torn by force beyond what they could take. And the moment it usually happens isn't the moment you'd guess — it's while the muscle is being stretched under load, not while it's shortening.
